Research Abstract

植込み型除細動器は心室細動(VF)などの致死性不整脈に対して極めて高い有効性を持つが、高エネルギーショックによる心筋傷害は低心機能脈者にとっては時に致命的になる。このため、より低エネルギーの通電による新しい除細動法の開発が求められている。一方、VFの主な機序は渦巻き様に旋回する興奮波(SW:spiral wave, scroll wave)であることが明らかとなっている。このようなSWは心臓の電気現象以外にも広く自然界で観察され、一定の条件下であれば外部からの刺激によってその挙動をコントロールできることか示されている。本研究では、VF中のSWを脱分極閾値以下の低エネルギー通電やペーシングによってコントロールしてVFを停止させる新しい除細動方法を開発する。
コンピュータ・シミュレーションにより、心室モデル内に誘発したSWを局所ペーシングおよび低エネルギー反復通電により停止させる方法を検討した。心室壁厚を菲薄化させた2次元的心室モデルにおいて、心室自由壁に安定に定在するSWは局所ペーシングおよび低エネルギー反復通電により停止することがあった。しかしながら、SWが右室左室接合部などの3次元的構造を有する部位に存在したり、SWが分裂するイオンチャンネル電流の設定や心室壁厚を菲薄化させない3次元的心室モデルでは、SWを停止させることは困難であった。シミュレーションから、局所ペーシングおよび低エネルギー反復通電による停止効果は、媒質の3次元的構造や不均一性、心筋の活動電位の特性等に極めて敏感であり、実際の生体への応用は極めて困難と考えられた。
実験的にも、2次元培養心筋標本上に安定して存在するSWはペーシングにより停止させることができたが、左室壁から切り出した3次元的心筋切片や心室壁を2次元的に菲薄化させたウサギのLangendorff灌流心標本に誘発したSWは停止させることが困難であった。
