Research Abstract

切開・蒸散作用の大きなEr:YAGレーザ(波長2.94μm)は、最近、医療の各診療科目に急速にその応用が展開されている。それは中空ファイバにより、Er:YAGレーザ光の安定伝送が可能になったからである。一方、止血作用の大きなNd:YAGレーザを併用することにより、患者に負担をかけない最小侵襲治療の実現が可能となる。それ故、種々の患者照射に対応し、術者にフレンドリーなレーザ照射素子を開発することは重要な課題になっている。本研究では以下の研究を行った。
1.先端装置一体型中空ファイバの効率的な製作法の研究
先端装置の効率的な製作法の研究を行い、ディスポーザブル使用に耐ええるような装置の製作について検討を行った。酸素ガスバーナーを用いた簡易な手法で、中空ファイバ母材石英端面部が溶融され、レーザ光を収束・発散、あるいは周方向や特定方向にのみレーザ光を放射させる内径0.7mm〜0.32mmのファイバ一体型素子を製作することが出来た。先端溶融部の付加損失は内径0.32mmファイバで2.3dB程度となった。
2.パイロット光付加型レーザバイポーラの研究
低侵襲の非常に細い血管部位を凝固、切断を1回のレーザ照射で行うレーザバイポーラを実現するための検討を行った。軟組織として牛肉を用い、凝固用としてNd:YAGレーザ光(最大照射エネルギー10mJ)を照射したがエネルギーが小さく、効率よい凝固効果が得られなかった。そこで高出力のHo:YAGレーザ光(波長2.1μm、最大照射エネルギー100mJ)を用い、明確な凝固効果を確認した。溶融部が石英ガラスに着目し、可視パイロット光、Ho:YAGレーザ、Er:YAGレーザ光を用いるレーザバイポーラを製作した。切開用と凝固用レーザ光の照射条件に対する軟組織の切開・凝固特性を明らかにした。
