Research Abstract

本研究では,内皮由来血管弛緩反応に伴って中膜が柔らかくなる現象を,超音波を用いて皮膚から直接計測するための高精度計測法を新たに開発し,内皮の障害も含め,動脈硬化症の無徴候性段階での検出も含めた動脈硬化症の極早期診断法の確立を目指している.
1.1拍内での血管壁の『応力-歪みヒステリシス特性』の評価を可能とする手法とシステムの開発を行った.(1)前年度に開発した連続計測システムを用いて,右腕の血管壁の1拍中での数μmという僅かな厚み変化を80秒間計測した.
同時に左腕の同じ位置で連続計測する血圧波形から,血管壁の1拍での『応力-歪み特性』を算出し,そのヒステリシス特性から動脈壁の弾性特性・粘性特性を評価した.(2)in vivo計測を実施し,駆血解除後の血管反応性に伴い,血管壁(平滑筋からなる内膜・中膜領域)での弾性値・粘性値が,60秒間連続計測できる計測システムを構築した.
2.ヒトの生理的刺激に対する血管内皮機能を評価した.上記のシステムを健常者に適用して血管反応性を評価したところ,血管駆血前の応力-歪みヒステリシス特性に対し,駆血解除直後においてヒステリシスが太くなって粘性が増加し,同時にヒステリシスの傾きが減少して弾性値も減少する現象,その約60秒後には,もとの駆血前の状態に戻る様子が観察された.これは生理学的にも説明ができる結果であり,これらの現象を非侵襲的に世界で初めて計測可能とした成果には大きな意義がある.
3.駆血をせずに血管内皮機能を評価するための方法の新たな開発をするため,自転車漕ぎによって血流を増加させたときの血管壁での内皮由来血管弛緩反応が検出できるかを調べたが,計測対象の微小な厚み変化に比べ,手の動きが大きく,これらの計測の精度を上げることは今後の課題である.
