院内感染発生の定量的予測システムの開発

Research Abstract

本研究は院内感染の発生リスクを入院時に定量的に予測するシステムの開発を行い、院内感染が顕在化・蔓延化する前の対応を可能にすることを目的とする。また同時に、医療現場に広く導入されつつある電子カルテ上のリアルタイムに得られる情報を医療安全に応用することも目指している。平成19年度の研究では、倫理委員会で承認された教育病院における入院患者について、入院後に発生した院内感染症(呼吸器感染症、尿路感染症、血流感染症)について、患者背景と共に臨床疫学的解析を行った。それらの結果を用いて、複数のモデルを作成し、最終予測モデルに必要な患者情報の抽出を行っている。今後、平成18年度から19年度にかけての研究結果を元に、日本における院内感染症の予測モデルについて、報告する予定である。本年度は、電子カルテ情報を用いた医療安全指標を定量的に予測するシステムの新しい展開として、入院患者における転倒の予測モデルも開発した。この予測モデルは、個別の患者における転倒のリスクを定量的に評価するのみならず、病院全体や個別の病棟におけるリスクを定量的に予測し、病院の安全性のトレンドや医療安全対策の有効性の評価にも利用できることが明らかになった。これら複数のアウトカムをターゲットにした定量的予測システムは医療機関における医療安全指標としても利用できる可能性を明らかにした。今後、医療安全領域において、電子カルテが持つ即時性の有用性を評価する予定である。
