Research Abstract

これまでに我々は、世界に先駆けてRunx3ノックアウトマウスを作製し、その解析よりRUNX3遺伝子が胃粘膜の発生や分化に重要な役割をはたしており、この異常が胃粘膜の脱分化や異常増殖や癌化に関連することを示した。さらに胃癌細胞株及び臨床検体におけるRUNX3遺伝子の高頻度のコピー数減少、主にメチル化による発現低下、機能喪失型変異を確認しており、RUNX3が胃癌の新規がん抑制遺伝子であることを報告した(Lie, Ito, Sakakura, Fukamachi,(equallycontributed)et al., Cell, 2002)。
また胃癌の発生母地である慢性胃炎粘膜や腸上皮化生におけるRUNX3の解析や胃癌の転移浸潤におけるRUNX3の関与を明らかとし、将来的にはこれらに基づく胃粘膜の発ガンリスク予測、胃癌の発ガン予防や消化管粘膜再生にも応用しると考えている。
RUNX3ノックアウトマウスの胃粘膜を経時的に観察すると、通常のmucous neck cellからの分化異常が認められ、Muc陽性、PCNA陽性、CDX2陽性の細胞が多数認められた。これらの分化異常を来した細胞群が胃癌の前癌病変である可能性が示唆された。これらの結果を基づきヒト胃癌臨床検体における胃癌の前駆細胞がヒト胃粘膜、特に癌部周囲に存在するかを検索する。これらを経時的に経過観察し、実際に発癌するか否かを確認する。さらにこれらを指標とした胃癌の発ガンリスク評価を多施設によるコホート研究にて行う予定である。
