Research Abstract

神経栄養因子の一つであるBDNFは脊髄後根神経節(DRG)において産生された後,順行性に脊髄後角に運ばれ,痛み受容線維の神経伝達修飾物質として働いている.BDNFはDRGでは通常TrkAを有する小型ニューロンに発現しているが,末梢炎症では小型ニューロン,完全軸索切断では大型ニューロンでの発現が増加するようになる.我々はこれまでに様々な慢性痛モデルを用いて,一次知覚ニューロンにおけるBDNF遺伝子発現の変化と慢性疼痛との関連を追及してきた.部分的軸索損傷モデルである神経因性疼痛モデルの傷害DRGニューロンにおいては,ERK1/2,ERK5,p38MAPK,及びJNKすべての活性化がみられるが,非傷害DRGニューロンにおいてはp38MAPKとERK5のみ活性化されることを見いだした.特に,非傷害小型DRGニューロンにおけるp38MAPKとERK5の活性化はBDNFやTRPチャネルの発現上昇を介した熱性痛覚過敏に関与していることが分かった.一方,傷害を受けた大型DRGニューロンにおけるERK1/2とERK5の活性化はBDNFの発現上昇を介したアロデイニアに関与していることが分かった.神経栄養因子の受容体一つであるp75NTRも同様に,非傷害DRGニューロンにおいて発現が上昇し,熱性痛覚過敏に関与していることが分かった.また,GFR alphalやalpha3などの受容体も傷害・非傷害DRGニューロンにおいて発現が上昇し,熱性痛覚過敏などの神経因性疼痛の発現・維持に関与していることが分かりつつある。このようにDRGニューロンにおける神経栄養因子やその受容体は様々な病的状況下において,異なった細胞内シグナル伝達によってその発現を調節されており,これらの発現変化が慢性痛の発生に関与していることが明らかになった.これらの成果はJ.Neurochem.等,数多くの国際誌に発表した.
