Research Abstract

本研究では高齢者の転倒防止を目指して、外乱負荷が加わったときのステッピング方略(代償的ステップ動作)が転倒回避に有効に作用するためには何が必要であるか、また、それはいかなるトレーニングによって増強するかを明らかにすることを目的とした。易転倒性高齢者は、以下のような代償的ステップ特性を有していた。前方手伸ばし動作によって、姿勢保持限界に到達する距離が短く、代償的ステップが誘発されやすい。代償的ステップ幅は小さく、ステップ高は低い。また、つま先から着床し、着床後の足圧中心は左右方向への動揺が顕著であった。代償的ステップによる姿勢制御には複数のステップを要し、前方へ移動した身体重心の制御に時間を要する。また、代償的ステップ時に体幹を起こすことができない。高齢者に週1〜2回のエクササイズ(歩行・筋力トレーニング、踊り:約60分)を3ヶ月継続した結果、ステップ幅は有意に改善したが、その他の易転倒性高齢者のステップ特性の多くは改善されなかった。
