Research Abstract

抗生物質の過剰使用は耐性菌の蔓延をまねく。抗生物質が過剰使用されているとされる状況はウイルスが原因の風邪に対する物が多いとされるが、中立なデータ源からのわが国における検証はこれからである。本研究ではある企業健康保険組合のデータの使用許可を得て、有る特定の3ヶ月の期間において特に細菌性と記されていない上気道炎の診断で受診した患者への抗生物質の処方割合を検討した。2577の受診が対象となりで60%の上気道炎受診で抗生物質が使用されていることがわかった。総合すると診療所の方が病院よりも処方割合は高く、処方割合を基準に施設数の分布を検証したところ2峰性となり、ほぼ全例に抗生物質処方する施設と、症例を選んで処方する施設の分布の差が見られた形となった。
さらに、Web質問法を用いて一般人の意識調査を行ったところ、半数以上がかぜで受診した際には医師に抗生物質を処方してほしいと思うと答えた。このように一般人の意識としてかぜに対しても抗生物質が有効であるという誤解が蔓延していることが明らかになり、今後抗生物質の過剰使用への対策の一つとして、一般人への教育が必要と考えられる。
