Research Abstract

本研究課題の目的は、従来の酵素抗体法と比較して高感度で微量物質の測定に極めて優れる,電気化学発光免疫測定法(Electrochemiluminescence immunoassay:ECLIA)を用いることにより、Gefitinibを代表とするEGFR-TKIに対する治療効果ならびに患者の予後を高率に予測可能な血清診断法の開発を行うことである。
本成18年度までに申請者の研究室で開発し, 現在,間質性肺炎の診断と疾患活動性の血清マーカーとして我国の医療保険で診断薬として承認されているKL-6 分子に反応するオリジナルのモノクローナル抗体を用いてECLLAを用いた測定系を構築した。KL-6は膜タンパク質であるMUC1ムチンの一部を詩織するものであり,間質性肺炎め診断や活動性に有用なマーカーとして臨床応用されているが,本年度はKL-6には多様な抗原性を示す多様な亜分子が存在し,肺癌特異性を高めたKL-6亜分子としてSLAK(セレクチシリガンドを有するKL-6亜分子:CA19-9とKL-6でサンドイッチされる分子)を見出し,SLAKはKL-6よりも強力な肺癌の予後因子であることを報告した(Inata J, et. al. Int J Cancer, 2007)一方で,MUC1はEGFRと結合すること,MUC1の発見がEGFR依存性のシグナル伝達を促進することが報告されている。本年度は,血清KL-6値のEGFR-TK1治療における有用性を検討を行い,血清KL-6値がEFGR-TKIに体する治療効果ならびに予後の治療前予測に有用であることを明らかとした(Ishikawa N, et. al.,Int J Cancer,2008)。さらにEGFR-TKIによる治療開始後2週間での血清KL-6値変化が,EGFR-TK1治療の反応性ならびに癌の再燃の早期検出に有効であることを明らかとした。続いて,ルテニウムと抗体の結合比率に関して,概ね至摘条件を設定することが出来た。今後は,SLAK値のEGFR-TKI治療における有用性の検討を行う予定である。
