乳歯齲蝕の発症関連要因及び予防要因の解明に関する前向きコホート研究

Research Abstract

平成19年12月末まで、3歳6か月時追跡調査を実施した。494組の母子が参加した(追跡率49.3%)3歳6か月時追跡調査では、本研究用に開発した質問調査票及び幼児用簡易版食事歴法質問調査票を用い、対象小児の睡眠状況、受動喫煙、テレビ視聴時間、保育状況、入浴、栄養摂取状況(母乳、人工乳、ベビーフード、肉、野菜、果物、糖類、食物繊維、ビタミン、ミネラル等)間食習慣、噛む習慣、歯磨き習慣、フッ化物応用状況、歯科受診状況、感染症既往等、生活習慣、生活環境に関して広範囲にわたる情報を得た。対象母子の口腔衛生状況を把握するため、調査スタッフ(歯科衛生士)が個別に訪問して口腔内を観察し、子のう蝕の有無、母のう蝕の有無及び歯肉の状態についての客観的なデータを得た。さらに、唾液流出量(母のみ)唾液の緩衝能(母子とも)及び口腔内のミュータンス菌量を半定量的に測定した(母子とも)3歳6か月時追跡調査で得られたデータのチェックと入力を実施した。研究事務局スタッフにより記入漏れの確認を徹底しているため、欠損データは殆どない。今後、今回の3歳6か月時追跡調査で得られた小児のう蝕を結果因子とし、過去の4回の調査(ベースライン調査、4か月時追跡調査、1歳6か月時追跡調査2歳6か月時追跡調査)で得られた胎児期および出生後の栄養状況や生活環境・生活習慣との関連について解析し、順次、論文を執筆する。これまで報告のある、出生時体重、母乳栄養、受動喫煙等と乳歯う蝕との関連について、日本人で初めて報告できる。
