Outline of Annual Research Achievements

申請者らは低分子有機化合物のプロトポルフィリンIX（PpIX）がX線との物理化学的反応により活性酸素の生成を増強する現象を見出し、光の替わりにX線を用いる「放射線力学療法（RDT）」の臨床実用化を目指した研究開発を進めてきた。これまでに、腫瘍選択的にPpIXの蓄積を誘起する5-アミノレブリン酸（5-ALA）を放射線増感剤として用いる「5-ALA RDT」が、X線単独よりも効率良く腫瘍細胞を損傷し、高い腫瘍増殖抑制効果や線量低減効果を示すことを、各種担癌モデルを用いて検証してきた。
X線照射により活性酸素の生成を増強するPpIX以外の有機化合物が存在し、また特定の化合物に注目すると活性酸素生成は化学構造と関係があること等が明らかになってきたが、詳細な反応メカニズムは未だ不明なままである。反応仮説の一つに、8M以上のリニアックの高エネルギー荷電粒子照射で生じるチェレンコフ光が、PpIXを光励起し、光化学反応によりROSを生じさせるというものがある。一方、チェレンコフ光が生じ得ない100～160kVの放射線照射装置でROSの生成を確認している。近年報告された術中放射線治療装置(IORT)の脳腫瘍に対する臨床報告において、5-ALAを用いた術中診断の直後に放射線を照射すると、放射線治療効果が増強されるという結果が示された。これは、PpIXが腫瘍に蓄積した状態で術中放射線治療を行うと、その効果が向上することを示唆するものであった。IORTのビームエネルギーは50kV程度と低い。そこでX線のエネルギー強度や線量率の影響についての詳細な検討を行った結果、線量率の増加に伴いヒドロキシラジカル生成は増加傾向を示す事、スーパーオキサイド生成は線量率に依存しない傾向を有すること等が確認された。
