Outline of Annual Research Achievements

本研究課題では、基礎研究の視点から、「移植再生医療リハビリテーション」を創設、提案、開発する。物理療法と運動療法を組み合わせるリハビリテーション法を検証し、最も効果的な方法を確立することを目的とする。そのため末梢神経の再生リハビリテーションについて【研究1】と【研究2】の実験を行い、超音波照射の影響を検証した。作成したトレッドミル歩行プロトコルを用い、運動機能評価を行った。
【研究1】では坐骨神経挫滅モデルラット32匹を、超音波照射を損傷1日後、3日後、7日後から超音波照射を開始する各群と擬似照射群の4群に振り分けた。超音波照射設定は周波数1 MHz、強度140 mW/cm2、時間照射率20%で1日5分の介入とし、これを4週間実施した。評価は損傷後1週ごとにトレッドミル歩行運動中の三次元動作解析による運動機能評価とした。【研究2】では同モデルラット36匹を別に作成し【研究1】と同じ実験を行い、運動機能評価に加えて損傷後4週の時点で電気生理検査と筋湿重量評価、電子顕微鏡観察を用いた組織形態学測定を実施した。
他にも、骨に対する電気刺激、関節軟骨に対する免荷刺激と超音波刺激の研究も実施した。現在、得られた結果を解析中である。
これら研究に関連する研究資料収集のため、および得られた研究成果を発表するため学会等に、主としてWebオンラインおよびオンデマンドで出席した。

Reason

本研究の当初計画は、①神経再生リハビリテーションについて、動物モデルで検証すること、②バイオ3Dプリンターを用いて人工神経導管を作成し、超音波刺激や電気刺激などの物理的刺激との相乗治療効果を検証することであった。
①については【研究1】と【研究2】において、ラット坐骨神経挫滅モデルを作成し、運動機能評価、電気生理検査、筋湿重量評価、電子顕微鏡観察を用いた組織形態学測定を実施した。②については継続して検討中である。③他にも、骨に対する電気刺激、関節軟骨に対する免荷刺激と超音波刺激の研究も実施し、現在、得られた結果を解析中である。

Strategy for Future Research Activity

3Dプリンターを用いた人工神経導管作成の研究の方向性について、今後検討する。また関節圧迫によるモデルラットの作成と運動介入効果の検証、および関節への負荷を減弱させる介入も検討する。
