Outline of Annual Research Achievements

本研究では、ワクチン接種率が向上した日本おいて、ワクチン予防可能疾患の潜在的な感染リスクグループを特定し、集団免疫度を評価することを主たる目標としている。第2年度目は(1)日本における感染性関連死の長期時系列トレンド解析、(2)日本における麻疹流行動態のメカニズムの解明、(3)日本における風疹流行の地理空間分析について取り組んだ。

(1)近年の日本では、衛生環境の改善や予防接種の普及などによって感染症を原因とした小児死亡が減少したが、高齢人口が増大したことにより、特に60歳以上高齢者の感染症関連死が増加傾向にあり、特に肺炎、敗血症、ウイルス性肝炎、腸管感染症、インフルエンザなどの死因が増加していることを特定した。また、感染症を起因とした子宮癌や肝臓癌も増加傾向にあることを明らかにした。また、肺炎球菌ワクチンの定期接種が死亡トレンドに与えた変化を検証した。
(2)麻疹は空気感染することから感染力が強く、ワクチンの集団接種以前においては、生まれたら必ず感染する感染症であると考えられている。感染者には強い免疫を残すため、流行規模は出生率(新規感受性者の数)によって変わることが知られている。日本における麻疹の長期時系列流行データを用い、出生率とワクチン接種率、季節性、を加味した数理モデルを構築し、周期的な流行を繰り返す麻疹流行動態のメカニズムが説明可能であることを明らかにした。
(3)風疹に対するワクチンの集団接種が不十分であると感染年齢が上昇し、平均感染年齢が妊娠可能年齢に達すると、集団における先天性風疹症候群のリスクが上昇する。集団免疫達成間近の日本では、これまでの巨視、画一的でランダムな予防接種では集団免疫の達成は困難である。したがって、年齢、性別、空間を加味し、どんな社会集団がハイリスクなのか特定し、どこを叩けば流行が減衰し十分な免疫割合に達するか定量化を試みている。
