Outline of Annual Research Achievements

本研究は、筋力（握力）の発達を手がかりに、乳幼児の健やかな運動発達の決定要因を明らかにすることを目的として行った。ニュージーランドに渡航し、32週以上で出生した早産児を対象とした栄養の無作為化比較試験であるDIAMOND trialの一部としてデータ収集を行った。そのうち、2歳（修正月齢）時点の追跡調査において握力を測定できた103名を対象として、握力と、母親の教育歴・民族性、社会経済状況、生後半年までの栄養法（完全母乳に至るまでの期間、退院時の栄養法、排他的母乳栄養の期間）、出生・退院・生後4か月・2歳時点での身体発育（身長、体重、除脂肪体重）、2歳時点の発達（Bayley Scale of Infant & Toddler Development-Ⅲを用いて評価した認知、受容・表出コミュニケーション、微細・粗大運動の得点）との関連を検討した。その結果、握力は、母親の教育歴・社会経済状況・2歳時点の身長が低いほど、また母親がPacific Islanderの場合に低かった。発達との関連では、握力と粗大運動の得点との関係のみが交絡因子調整後も有意であった。この結果は、乳幼児の粗大運動の発達における筋力の関与と、筋力の発達に対する出生前要因の寄与を示しており、子どもの健やかな運動発達には出生前要因も考慮した対策の必要性が示唆される。現在、本研究成果を英文誌に投稿すべく論文執筆をすすめている。
この他に、渡航前に日本で取得したデータを用いて、乳幼児期の運動発達と将来的な肥満に関する後ろ向き追跡研究を行い、ハイハイ、つかまり立ち、つたい歩きの達成時期が遅い児は、小学1年時の体脂肪率が高いことを明らかにした。子どもの健やかな運動発達が、生涯にわたる健康にとって重要な役割を果たすことを示した本研究成果は、Maternal and Child Health Journalに掲載された。
