Outline of Annual Research Achievements

本年度は、胎児心エコー検査によるストレイン計測を胎児不整脈や胎児心機能の診断に応用する研究を進めた。
胎児不整脈の診断には、Mモードでの心房・心室壁運動記録、上大静脈・上行大動脈のパルスドプラ波形同時記録が広く用いられているが、胎位によっては適切な断面・カーソル位置の設定が難しいことがある。そこで、ストレイン解析を応用して、胎児徐脈性不整脈の診断ができないか検討した。まず、不整脈のない18症例の胎児心エコー四腔断面像を用いてストレイン解析を行った。左室と右房の心内膜をトレースして連続3心拍のストレイン曲線およびストレインレート曲線を得て、正常リズムにおける心周期の時相を同定した。その結果、正常リズムの胎児では、左室のストレインレート曲線上で、急速駆出期に相当する収縮期ストレインレートが最大となる時相(S波)が同定でき、また右房のストレインレート曲線では、拡張期に心房収縮期に相当する大きなピーク(A波)が同定できた。次に、徐脈性不整脈の3症例も同様に解析したところ、いずれも特徴的なS波とA波の関係がみられ、そこから洞性徐脈、ブロックを伴う心房性期外収縮、完全房室ブロックと診断することが可能であることが分かった。胎児不整脈の診断の新たな手法として、ストレイン解析が有用であることが示せたと考えている。
胎児心機能については本邦初となる重症胎児大動脈弁狭窄症への胎児治療の症例に対して、治療前後の心室や心房のストレイン解析を行った。胎児大動脈弁をバルーンで拡張する治療により、左室の流入血流は二峰性に変化し、それに伴って胎児の左心室および左心房のストレインも変化し、収縮能および拡張能の改善が認められた。研究代表者の退職、医院開業に伴い、研究は後任の若手医師に引き継がれ、現在詳細な解析を行っているところである。
