Outline of Final Research Achievements |
Risk factors for posttransplant recurrence of focal segmental glomerulosclerosis (FSGS) have not been established. We examined risk factors for posttransplant recurrence in 36 FSGS patients aged 1-25 years at FSGS onset from seven institutions in Japan, excluding those with secondary FSGS, familial/syndromic FSGS, and genetic FSGS. Our study suggested that treatment responses to initial steroid therapy and/or additional therapies such as immunosuppressive agents, rituximab, plasmapheresis, and/or LDL apheresis may predict posttransplant recurrence of FSGS.

Academic Significance and Societal Importance of the Research Achievements |
FSGSはステロイド抵抗性・難治性ネフローゼ症候群を呈しながら末期腎不全に進行し、さらに、腎移植後、高率に再発して移植腎機能廃絶に至る希少難病である。小児では末期腎不全の原因疾患として2番目に多く、また移植腎の生着を妨げる重大な要因であるため、FSGSの腎移植後再発は国際的な最重要課題である。しかしながら、腎移植前のFSGSの再発予測法は確立していない。本研究では、詳細な臨床病理学的検討と網羅的な遺伝子解析を加えた検討で、発症後のステロイド、免疫抑制薬、リツキシマブ、血漿交換、LDL吸着への治療反応性が腎移植後再発を予測するうえで有用な情報となりうることが示された。
