Outline of Research at the Start

大動脈解離は命に関わる疾患であるが、予防法はなく、有効な内科的治療法もない。治療に役立つ知見をえるため、分子的な病態解明が急務である。本研究では大動脈を構成する平滑筋と、代表的な炎症細胞であり解離との関連が示唆されているマクロファージに注目し、解離発症に先立ってこれらの細胞がどのように相互作用をもっているのかを解明する。

Outline of Final Research Achievements

This study investigated how perivascular adipose tissue (PVAT) senses triggers of aortic dissection (AD) and promotes inflammation in non-hereditary AD. Using a mouse model, spatial transcriptomics and gene expression analyses revealed phenotypic changes and increased inflammation in PVAT and medial smooth muscle layer after BAPN+AngII infusion. Importantly, these alterations appeared even before the onset of dissection, indicating subtle cellular interactions precede disease development. These findings provide new insights into early AD mechanisms and may guide future therapeutic target discovery.

Academic Significance and Societal Importance of the Research Achievements

大動脈解離は中年以上に好発する致死的疾患で、効果的な内科療法がないことは高齢社会においてさらに問題となっている。解離モデル動物が普及し分子病態研究も進む中、炎症応答が中心であることはわかってきたが、大動脈組織が炎症を感知し激烈な組織破壊にまで増幅する機構は不明である。本研究は大動脈をとりまく脂肪組織に着目し、その細胞形質の変化が炎症応答を惹起する可能性を示した。脂肪細胞は炎症を惹起する一方で形質によっては抗炎症作用を持つことが知られており、内科的治療応用へ向けてさらなるメカニズム解明が期待される。
