Outline of Final Research Achievements |
For the purpose of establishing a triage system for elderly home care patients, we developed the "Advanced Care Plan-based Emergency Contact Form" as a framework to facilitate cooperation among elderly care facilities, fire departments, and emergency medical institutions.. In the future, we will promote the Verification Committee for a triage system for elderly home care patients by verifying the effectiveness of the contact sheet that has actually been put into operation.

Academic Significance and Societal Importance of the Research Achievements |
人生100年時代という超高齢社会において,特に人生の終末期に『患者の意向を尊重した』医療を行うためのAdvance Care Planning:(ACP)の概念が普及しつつあるが,救急対応に関する地域での議論は十分にできてはいない現状がある。本研究の成果により人生終末期の救急医療に関する意思表示の方法,Advanced“Emergency”Care Planningについて検討し、住民が住み慣れた地域で自分らしい暮らしを,人生の最期まで続けることができるようにするために,地域の在宅医療・介護と救急医療の有機的連携ができるようなシステムの構築が期待できる。
