Outline of Final Research Achievements

We targeted elderly patients with moderate to severe obstructive sleep apnea(OSA) who required and started continuous positive airway pressure(CPAP) but withdrew the treatment, to seek for an alternative treatment for OSA. We devised 3 kinds of new myofunctional therapy(MFT) which trains upper airway muscles, and they all consisted of simple movements that elderly patients would not find difficult to continue everyday.
However, only 6 patients met the criteria for our study. 2 patients were enrolled , but both patients withdrew MFT as they found it troublesome to do the exercise everyday.

Academic Significance and Societal Importance of the Research Achievements

現在中等度以上のOSAに対する治療の第一選択はCPAPであるが、CPAPは特に高齢者においては一般的には装用時違和感等から離脱者が多い、原則として生涯続ける必要がある、心血管予後を改善しないことが示唆されている等の問題点があり、CPAPに代わる治療が模索されてきた。
使用時間帯のみ効果を発揮するCPAPとは異なり、上気道開大筋群を鍛えるMFTは能動的にOSAを改善し、就寝全時間帯での効果が持続することが期待される。
またMFTのOSAに対する効果が一定期間残存する、或いは長期的に心血管リスクを低減する可能性もあり、今後の研究結果次第ではCPAPに代わるOSA治療の第一選択となる可能性も考えられる。
