Outline of Final Research Achievements |
This study attempted to support the self-management of patients with ischemic heart disease through the combined use of tele-nursing. In the process, measures to deliver nursing care to a larger number of subjects, effective support frameworks, and situations that make self-management difficult for patients with ischemic heart disease were examined. Due to the limitations of the individual applications in this study, we proceeded with the development of a platform but were unable to complete it within the study period. On the other hand, we confirmed that the "powerlessness" that makes self-management difficult is a lack of a sense of control and an inability to make self-decisions, and found that self-management and "living with heart disease" can be supported by increasing health literacy and empowerment of patients with heart disease. The findings are as follows.

Academic Significance and Societal Importance of the Research Achievements |
虚血性心疾患は生活習慣に起因する心疾患であり、心不全の連続した病態の一つとして位置づけられている。心不全は生活機能の低下と生活の質の低下につながるため、虚血性心疾患患者の自己管理を支援し、心不全発症を予防することは臨床的意義が高い。 本研究は、自己管理(セルフケア)の3つの主要概念から過去の患者の取り組みを分析し、アプリケーションの提供だけでは、その成果が患者のヘルスリテラシーに依存するという課題を明らかにした。さらに、既存のエンパワメントアプローチという自己管理の支援方法の目標と、慢性病とともに生きることにおける概念の一致を確認したことは、有効な心疾患患者の自己管理支援方法の開発の一助となる。
