Research Abstract

我々は、PI3K の活性化はβ細胞の機能・増殖に重要であり、その低下が2 型糖尿病におけるインスリン分泌不全に寄与していると仮説を立てた。これを証明するため膵β細胞特異的にPI3K の主要な調節サブユニットの一つであるpik3r1 遺伝子をノックアウトしたマウスβpik3r1KO を、またもう一つの主要な調節サブユニットpik3r2 null マウスとβpik3r1KO を交配させて、β細胞特異的な調節サブユニットの完全ノックアウト(βp85DKO)マウスを作成した。βpik3r1KO マウス、βp85DKO マウスでは、グルコース依存性のインスリン分泌障害のため、高度の耐糖能障害を呈しており、βp85DKO マウスでその障害はより顕著であった。このインスリン分泌の障害として、(1)PI3K の下流であるAkt の活性化の低下によりapoptosisが亢進しており、β細胞量の減少がβp85DKO マウスでは有意に認められた、(2)gap junction形成因子Connexin 36 の発現が有意に低下しており、 通常はグルコース刺激に対して認められるラ氏島全β細胞の同調的インスリン分泌が障害されていた、(3)インスリン分泌顆粒のexocytosis に重要なSNARE 蛋白複合体を形成する分子の発現がcluster として低下していることがcDNA マイクロアレイで確認された。多くの2 型糖尿病患者では、グルコース応答性のインスリン分泌ことに第一相の低下を呈し、周期的同調的インスリン分泌oscillationの低下や、経時的なβ細胞量の低下を伴う。従って、本研究の成果から、インスリンシグナルの下流においてPI3K が膵β細胞の機能や増殖に重要な役割を果たしており、2 型糖尿病ではβ細胞のインスリン抵抗性(インスリンシグナルの低下)が、β細胞の量や機能の低下をもたらして病態を修飾していることが示唆された。
