| Research Abstract |
Stevens-Johnson症候群(SJS)と中毒性表皮壊死融解症(TEN)の新規治療法開発を目的に、患者素因と病態を詳細に検討した。患者ではHLA-A*0206が有意に高頻度であり、TLR3遺伝子多型rs3775296、IL4R遺伝子多型rs1801275(Gln551Arg)、FasL遺伝子多型rs. 3830150に有意な相関を認めた。IL13遺伝子多型rs20541(Arg110Gln)も有意な相関を認め、IL4R遺伝子多型Gln551GlnとIL13遺伝子多型Arg110Argの両方をもっ割合が患者で有意に高かった。眼合併症を伴うSJS/TENは、口唇の発疹、爪周炎、皮疹に先行ないし同時に生じる結膜充血が特徴である。発症早期のステロイドパルスと眼局所ベタメタゾンによる消炎が角膜上皮幹細胞の喪失と眼表面瘢痕化を防ぐために有用であった。
