Research Abstract

浸潤性乳管癌における腫瘍内Fibrotic focus(FF、線維化巣)の存在は、浸潤性乳管癌患者の短期・長期予後を考える上で非常に重要な組織形態学的因子であることを後ろ向き研究並びに前向き研究により既に証明した。平成19年度では、FFの存在が術後加療なし群では、リンパ節転移陰性群、pTNM-I/II群において、FF径、FF線維性状が遠隔臓器転移再発、骨転移再発、肝転移再発と密接に関係していた。術後加療あり群では、リンパ節転移、pTNM病期に関係なく、FF径が遠隔臓器再発、骨転移再発、肝転移再発と密接に関係し、腫瘍壊死、N3(リンパ節転移個数10個以上群)が肺、肝転移と各々有意に相関した。研究の結果、FF性状は術後加療、リンパ節転移、pTNM病期に関係なく、遠隔臓器、特に骨、肝転移再発と密接に関係していることが明らかとなった。平成20年度では、FFに加え乳管癌リンパ管腫瘍塞栓を形成する腫瘍細胞の核分裂像、アポトーシス像に着眼したリンパ管組織異型度を考案した。393例の浸潤性乳管癌症例で検討した結果、術後加療の有無、リンパ節転移の有無、腫瘍径の違い等に関係なく、リンパ管腫瘍塞栓組織異型度は浸潤性乳管癌患者の腫瘍再発、腫瘍死と有意に関係することを既知臨床病理学的予後因子との多変量解析により明らかなものとした。この時、FFの存在もリンパ管腫瘍塞栓組織異型度と同様に優位な組織学的因子であることをあらためて確認した。
