Research Abstract

zinc finger型の転写因子glioma-associated oncogene homologue 1 (Gli-1)はHedgehog(Hh) signalling pathwayの重要な構成成分である。Hh経路はmorphogenesisやstem-cell renewal に必要不可欠であり、近年多くの癌腫においてその異常が報告されている。 食道癌の進展におけるGli-1発現の役割は未だ明らかにされておらず、今回、私達はneoadjuvant chemoradiotherapy (CRT)を施行した食道癌患者において、Gli-1発現が病期の進行や予後と関連するのか否かを検討した。病理組織学的に食道扁平上皮癌 (ESCCs)と確定診断された69 症例を対象とし、このうち25 症例は術前CRT にpathological complete response (pCR)を示した。 Overall survival (OS)はリンパ節転移の有無、遠隔転移の有無、CRTの反応性の有無と有意に相関し、更にGli-1 nuclear expressionおよび遺残腫瘍の有無とも相関した。 Gli-1 nuclear expressionを示した7症例(10.1%) は全例遠隔あるいはリンパ節転移を示し、その内6症例は13ヶ月以内に再発により死亡していた。Gli-1 nuclear-positive cancersはそうでない症例に比べて有意に予後が不良であった (Disease-Free Survival ; mean DFS time 250 vs 1738 days, 2-yr DFS 0% vs 54.9%, P=0.009, OS ; mean OS time 386 vs 1742 days, 2-yr OS 16.7% vs 54.9%, P=0.001)。 本研究によって、術前CRT施行ESCC患者においてGli-1 nuclear expressionは独立した信頼性の高い再発および予後の規定因子であることが明らかになった。以上の結果より、Hh経路の活性化がCRT後のcancer regrowth とprogressionを促進することが示唆された。
