Research Abstract

慢性腎臓病の主病変である腎間質線維化の機序を明らかにするため、抗炎症作用を有する脂質親和性受容体(グルココルチコイド受容体(GR)、ペルオキシゾーム増殖薬活性化受容体(PPAR))とそのリガンドの転送蛋白(脂肪酸結合蛋白 : FABP)の炎症・線維化調節作用を炎症刺激下、低酸素下のヒト近位尿細管上皮で解析した。GR活性化薬(グルココルチコイド)は、定常状態あるいは低酸素・TNF-α誘導性のPAI-1(線維化促進因子)産生を増強した。これは、チロシンキナーゼ経路を介しGR依存性であった。一方、同薬は、定常状態と低酸素誘導性のVEGF(血管新生因子)産生をGR非依存性に抑制した。PPARについては、尿細管細胞に機能的PPAR-γ蛋白発現を確認した。その活性化薬(ピオグリタゾン)は標的遺伝子であるFABP, LXR発現を誘導するとともに、ケモカイン産生を抑制するかたちで抗炎症作用を呈した。TNF-α, TGF-β刺激では、PPAR-γ発現とその標的遺伝子発現が抑制された。以上より、臨床で汎用されるグルココルチコイドには、抗炎症作用だけではなく、線維化促進作用、血管新生抑制作用が推察され、PPAR-γについては、障害腎におけるPPAR-γ経路の変容、減弱が推察された。
