Research Abstract

最近の統合失調症薬物療法の中心となっているのは、非定型抗精神病薬である。近年急速に伸展した脳神経画像研究によって、統合失調症に関しても、脳の特定部位の萎縮が薬物治療開始前から存在することや、病態の進行に伴う萎縮の拡大が指摘されている。統合失調症における脳萎縮の原因は不明であるが、代表的な神経変性疾患であるアルツハイマー病のように中枢神経における、中枢神経系の炎症機転が関与している可能性がある。実際に、抗精神病薬に抗炎症薬を付加することによって、統合失調症の治療効果が増強されたという報告も知られている。中枢神経系の炎症の機序には脳内マクロファージであるミクログリアが深く関わっている。ミクログリア活性化に対する非定型抗精神病薬の影響を検策することによって、統合失調症の発症や再燃の、中枢神経系の炎症を通じたメカニズムを探り、統合失調症の根本的治療法への道筋を明らかにすることをその目的として研究を行った。今回の実験結果より、非定型抗精神病薬(アリピプラゾール、クエチアピン、ペロスピロン)はLPSやIFN-γによって活性化されたミクログリア由来のフリーラジカル(nitric oxide)や炎症性サイトカイン(TNF-α)の産生を有意に抑制すること、及びその抑制機構には細胞内カルシウムイオン濃度調整が関与していることが示された。ミクログリア活性化は統合失調症脳内の器質性変化をもたらし、病態の慢性化・不可逆性化に関与する可能性があるとされている。従って、これらの薬剤は急性精神病状態の治療のみでなく、統合失調症の慢性化・不可逆性をも抑制し、陰性症状の発現や認知機能障害予防に効果がある可能性が示された。
