Research Abstract

PET/CTによる動脈のアテローム硬化病変の予測診断の可能性について、検討した.^(11)C-cholineおよび^(18)F-FDG(fluorodeoxyglucose)を用い、動脈壁への集積の程度と石灰化の関係について調べた.^(11)C-choline PET/CTおよび^(18)F-FDG PET/CTを施行された60歳以上の症例で、^(11)C-cholinePET/CTおよび^(18)F-FDG PET/CTともに90%以上の症例で、両側総頸動脈、上行大動脈、弓部大動脈、胸部下行大動脈、腹部大動脈壁に^(11)C-choline および^(18)F-FDGの集積が認められた.しかし動脈壁の石灰化に一致した集積が見られたのはわずかだった.BMI、年齢はSUV、石灰化との間に明らかな相関は見られなかった.以上の結果から、動脈壁の集積部位はアテローム硬化が石灰化に至る過程のどこかの課程に相当すると考えられた.また^(11)C-cholineと^(18)F-FDGの集積の程度を比較すると、 ^(18)F-FDGの集積の程度のほうが高いことが分かった.またドイツ人と日本人との比較ではドイツ人の方が動脈壁に集積する頻度が高いことが分かった.^(11)C-cholinePET/CTおよび^(18)F-FDG PET/CTともに動脈壁病変の予測診断にある程度使用できることが確認されたが、詳細な心血管病変との関係や微細な動脈硬化病変との関係については不明な点が多く、今後も検討を続けていきたい.
