Research Abstract

手術侵襲後の生体には恒常性を維持するべく炎症性サイトカイン産生を中心とした生体防御反応が起こるが, その反応が過剰であると感染症などの術後合併症が引き起こされる。われわれは, 消化器癌手術症例を対象として, 脂肪細胞特異的に産生され, 抗炎症性作用を有するアディポネクチンが術後炎症性生体反応に関与し, その侵襲後の変動が手術侵襲度に依存していること, さらに, 大腸癌症例を対象とした検討において, 術後感染症発生群では非発生群に比べ, 術前から血清アディポネクチン値が有意に低値であること, 術前アディポネクチン値の低下が術後感染症発生の独立した危険因子であること, その機序として, アディポネクチン産生の低下は抗炎症性作用の抑制から術後の過剰な炎症反応を惹起し, 易感染性が誘導されることを明らかにした。
