Research Abstract

本研究の目的は, 頭位, 体位, および下顎位を変化させることにより, 気道形態にどの様な影響を及ぼすかについて検討するとともに, 呼吸量についても計測し, これらの関係について明らかにすることにある.
今回, いびきや睡眠障害を認めない健康成人男性15名を被験者として, 習慣性閉口位(下顎0%前方位)と下顎最前方位(下顎100%前方位)の下顎位を記録した後, 下顎0%前方位と下顎75%前方位の2種のバイトブロックを作製した.被験者にバイトブロックを装着させ, 5体位(仰臥位と-下顎0%前方位, 仰臥位-下顎75%前方位, 仰臥位-側頭位-下顎75%前方位, 側臥位-下顎75%前方位, 腹臥位-側頭位-下顎75%前方位)における上気道MRIを撮像した. つぎにRealiaソフトウェアを用いて各上気道MRI画像を処理し, 上気道全体の3D模型を作製し, 上気道形態の変化について検討し, 仰臥位-側頭位-下顎75%前方位の場合が最も上気道容積が大となることがわかった. また, 側臥位-下顎75%前方位の場合が吸気量が最大となることがわかった. この結果は閉塞型睡眠時無呼吸症候群(OSAS)患者の睡眠時の呼吸をスムーズに保つ最適な姿勢の選択に参考となり, 患者の症状を軽減する方略として役立てることができると思われた.
