Research Abstract

膀胱移行上皮癌進展への分子機構については近年の分子生物学の発達により、多くの知見が得られてきた。表在性乳頭状癌の初期発生にはFGFR3やPIK3の変異、p16(染色体9p欠失)、TSC1変異、9q欠失などが示されている。浸潤癌の前駆病変とされるCISにはp53変異やRbなどの異常を伴い、CISから浸潤癌へは9pや9qの欠失を伴うことが多いとされてきた。これらは一見、TCCの発症進展が非常にバリエーションに富んだものと見られるが、近の分子生物学的な解析により細胞内シグナル伝達系から考えると、膀胱癌の発生・進展にはPI3K-AKT系の亢進が非常に重要である可能性がでてきた。
本研究ではTCCのバリエーションに富む遺伝子変異群のなかで浸潤癌へのキープレイヤーを遺伝子欠損あるいは導入マウスを用いて同定し、これをヒト膀胱癌に当てはめることが出来るかを検討し、治療に難渋する浸潤性膀胱癌のキープレイヤーとなる標的分子を同定する足がかりを見つけることを目的とする。
手始めに、われわれは血管新生に関与するHIF-1やアポトーシス制御に関するSurvivinに注目し、解析を進めた。その結果、HIF-1αの遺伝子多型が浸潤性膀胱癌の予後を非常に反映することを示し、報告した。
Survivinについては免疫染色や遺伝子多型解析を進め、このタンパク変異を伴う遺伝子多型が膀胱癌の発症に関与していることを示し、現在、このタンパク変異による機能変化を解析している。結果としてSurvivinのプロモーター多型は膀胱癌のリスクに影響し、また発現量にも反映される。Survivinのエクソン多型も膀胱癌の発症に関与していることが示唆されるが、その生物学的機能的意義は未定である。現在cDNAの発現ベクターによる多型の意義を生物学的に解析中である。また臨床検体を用いたSurvivinの発現と予後、治療反応性などについては、膀胱全摘除術後の再発やがん死と発現亢進が関連していることが示された。
