Research Abstract

統合失調症の主症状である思考障害や幻聴の病態には、言語処理の障害が深く関与しているものと考えられている。われわれは、統合失調症における幻覚妄想や思考障害の病態基盤を理解するために脳機能画像を用いて聴覚言語処理時の脳活動について健常人との比較検証をおこなった。統合失調症において、ヒトの声に含まれる感情の抑揚の理解時に右側頭葉の活動が低下し、幻覚妄想などの陽性症状が強いほど左前頭葉の活動が強くなり、感情の平板化や引きこもりなどの陰性症状が強いほど左側頭葉の活動が強くなることが確かめられた。これらの結果は、統合失調症の幻覚妄想の病態理解には言語処理時の大脳半球左右差の変化が重要であることを示唆しており、今後も脳画像と遺伝子要因との関連性も含めて統合失調症の個別化医療に役立つ臨床評価法の開発を継続していきたいと考えている。
