| Research Abstract |
全国の急性期病院を対象とした調査により, 急性期病院において地域連携および退院支援の体制整備が進められていること, ならびに退院支援担当の看護職の配置が進み, 退院支援担当の看護職が退院後も医療やケアを必要とする患者の在宅移行時に退院支援の中心となっていることが明らかとなった。これらより, 多職種が協働する退院支援において, 看護職は医療と生活の両面から患者を支援し, 在宅移行の重要な担い手になっていると推察された。
