Research Abstract

肺移植後急性期に生じる虚血再灌流障害(ischemia-reperfusion injury : IRI)のコントロールは、急性期の術後成績の改善のみならず、臨床肺移植の直面する2つの大きな課題であるドナー不足の解消と慢性期グラフト機能不全を克服するという点において非常に重要である。(Carbon Monoxide ; CO)は種々の細胞保護作用をもち、移植領域においてもCOの投与によるIRI抑制やグラフト生存期間の延長効果が示されているが、小動物モデルによる結果が殆ど全てである。COが有毒ガスとして知られることを考慮すると、臨床応用のためにはtranslational researchとして大動物を用いた検討は不可欠であり、今回の研究に着手した。この結果、ミニブタによる肺IRI モデルを確立し、安全性の高い250ppmCO吸入によってIRIが抑制されるということを示し、またこのCO吸入によるIRI抑制効果が特に血管内皮の保護効果、炎症性サイトカインの有意な産生抑制を介したものであることを示した。この実験結果をもとに、臨床肺移植に即したMHC完全不適合間ミニブタ左肺移植モデルを用いて、CO吸入の移植肺生存期間延長効果および慢性拒絶病変進展抑制効果の検討実験へと進むことが可能となった。
