Outline of Research at the Start

近年、がんの放射線療法において、アルファ線を用いた核医学治療が大きな注目を集めている。がんの浸潤・転移においては腫瘍全体の90%以上を占めるがん組織の間質が重要な役割を果たしており、特にfibroblast activating proteinα(FAP)は乳がん・大腸がん・膵がん・卵巣がん・肝細胞がん等の幅広いがんの間質での発現が認められている。本研究では、がん間質細胞の特異的リガンドであるFAPIをアルファ線核種のアスタチン(At-211)で標識することで、革新的なアルファ線治療のPOC(proof of concept)の取得を目指す。

Outline of Final Research Achievements

We evaluated nuclear medicine therapy targeting FAP (Fibroblast Activation Protein) expressed in cancer-associated fibroblasts (CAF). CAFs create the environment for cancer cell proliferation and also act as a barrier to prevent anticancer drugs from reaching cancer cells. To evaluate therapeutic efficacy, a study was carried out using FAPI-46 labelled with the α-ray nuclide actinium (Ac-225) and the β-ray nuclide lutetium (Lu-177). Ac-225 FAPI-46 showed a good initial therapeutic effect but was prone to re-proliferation in the later stages, while Lu-177 FAPI-46 showed a slow onset of therapeutic effect but a sustained therapeutic effect.

Academic Significance and Societal Importance of the Research Achievements

がん間質はがん細胞の増殖に必要な環境を作るとともに、がん細胞に抗がん剤が届くのを防ぐバリアにもなっている。特に膵臓がんでは、がん細胞の周囲に存在する間質がバリアとなって、がん細胞まで抗がん剤が行き渡らないことがあった。今後、通常の治療が効かない難治性の膵臓がんに対する画期的な治療法となることが期待される。さらにFAPは膵臓がん以外にも多くのがん種で発現していることがわかっている。今後、他のがん種に対する治療の有効性も検証し、がん治療戦略の1つに位置づけたい。
