Outline of Research at the Start

脊髄損傷や脳卒中患者に対するリハビリテーションとして、他者の動きを観察する「運動観察」と脳内での運動再現である「運動イメージ」に基づいた介入手法が用いられている。本研究の目的は、歩行を対象とした運動観察と運動イメージが脳や脊髄といった中枢神経系の活動に及ぼす影響を明らかにすることである。また、脳活動の情報から機械を操作するブレイン・マシン・インターフェースと、筋を電気刺激装置により収縮させる機能的電気刺激を、歩行の運動観察および運動イメージと組みわせて、新たな歩行のリハビリテーションの提案を目指す。

Outline of Annual Research Achievements

本研究は、歩行のリハビリテーションへの応用を見据えて、歩行観察と歩行イメージが中枢神経系の活動に与える影響を明らかにすることを目的とした。歩行観察とは他者の歩行を客観的に観察すること、歩行イメージは歩行中の筋感覚を脳内で想起することと定義される。これまでの一連の研究により、歩行観察とイメージを組み合わせた歩行観察+イメージを実施している際に、皮質脊髄路興奮性と脊髄運動ニューロン興奮性が増大すること(Kaneko et al., 2018, 2020)、感覚運動野が賦活すること(Kaneko et al., 2021)が明らかとなった。
今年度の研究では、リハビリテーションへの応用を見据えて、健常者を対象として、歩行観察+イメージを用いた20分間の単発介入が皮質脊髄路興奮性と脊髄運動ニューロン興奮性に及ぼす影響を調べた。介入後、被験者の歩行イメージ能力の向上が認められたが、興奮性の変化は認められなかった。次いで、歩行観察+イメージと神経筋電気刺激を組み合わせた介入効果の検証を行った。神経筋電気刺激は総腓骨神経と脛骨神経に対する電気刺激であり、前脛骨筋とヒラメ筋を収縮させた。刺激タイミングは観察する歩行動画と同期させた。併用介入前後における皮質脊髄路興奮性と脊髄運動ニューロン興奮性の変化を調べた。その結果、歩行観察+イメージと神経筋電気刺激を組み合わせた介入が皮質脊髄路興奮性を増大させることが明らかとなった。神経筋電気刺激を単独で使用した介入では、興奮性の変化は認められなかった。したがって、皮質脊髄路興奮性の増大は、歩行観察+イメージにおける感覚運動野の賦活と、神経筋電気刺激による感覚入力が同期することで起きたと考えられる。この興奮性の増大は、脊髄損傷患者の歩行機能の回復と正の相関が認められていることから(Thomas & Gorassini, 2005)、本研究の介入手法はリハビリテーションに有効である可能性がある。
