Outline of Research at the Start

ヒトの体幹筋はあらゆる身体運動において身体を支える役割を担う。腕を挙げるという動作でさえ、上肢運動と同時に体幹筋が姿勢保持をすることで初めて遂行可能となる。この場合動作の意識は腕を挙げることにあり、体幹筋の活動は意識されることはない。それにも関わらず、体幹筋は上肢の運動に協調し、適切なタイミング・強さで活動する。こうした体幹筋の活動を我々の脳や脊髄といった中枢神経系はどう制御しているのだろうか。本研究では車椅子アスリートを優れた体幹制御能力を有するモデルとし、種々の神経学的手法を用いその制御特性を明らかにすることで、優れた体幹制御を可能にする神経メカニズムを解明する。

Outline of Annual Research Achievements

前年度までは四肢の運動に協調して働く体幹筋の神経制御メカニズムに関して、上肢-体幹間の神経的結合性に注目し、その存在の有無や特性について健常者および、車椅子アスリートを対象に研究をしてきた。
最終年度ではこの神経的結合性がヒトにおいてどのような機能的意義を持つかという点に注目して検討した。特に、行われる運動の種類や強度に依存して、結合性を強める(あるいは弱める)変調が見られた場合、神経的結合性は筋間の協調的な運動に寄与している可能性が考えられた。前年度までに確立した経頭蓋磁気刺激(TMS)による神経的結合性の評価法を用いてこれを検証した。その結果、運動の種類や強度に依存して、上肢-体幹間の神経的結合の程度が変調し、さらに体幹筋の中でも屈筋(腹直筋)や伸筋(脊柱起立筋)など、機能の異なる筋では、上肢運動に対する神経活動の変調の程度が異なることも明らかとなった。この結果は上肢-体幹間における神経的結合は運動の種類や強度によってその結合強度が変わり、上肢-体幹間の協調運動に寄与している可能性を示唆した。この研究に関する成果は国際学術誌eNeuro誌に掲載された。
さらに上記の研究から発展して、TMSの2連発刺激などの手法を用いて、異なる筋間の協調運動に関与する皮質内の抑制・促通機構の働きをを明らかにした。この結果は論文にまとめ、国際学術誌への投稿準備をしている。
以上のように四肢と協調して働く体幹筋の制御に関して、その神経メカニズムをさまざまな観点から検討し、国際学術誌に発表するなど、当該研究を大きく進めることができた。
