Outline of Research at the Start

①膵臓癌微少環境へ腫瘍抗原(新規WT1 やネオアンチゲンなど)特異的細胞障害性T 細胞(CTL)を誘導することを目的とした臨床試験を実施・完了する。
②膵臓癌微少環境の免疫状況(亢進または抑制状況なのか)を、適切なタイミングで予測できる簡便な免疫バイオマーカーに関して、詳細かつ網羅的な探索を行う。
③免疫バイオマーカーによる「膵臓癌微少環境の免疫状況」の予測結果から、「個別化複合的免疫化学療法」の治療選択基準を提案する。

Outline of Final Research Achievements

(1) Clinical trial on standard chemotherapy combined with a novel WT1-piulsed dendritic cell vaccine was conducted. We identified a Neo-WT1 peptide cocktail that can stimulate WT1-specific CD4 and CD8 T cells for advanced pancreatic cancer, and developed dendritic cell vaccines therapy (Neo-WT1-DC) pulsed with the Neo-WT1 peptide cocktail.

(2) Clinical trial on standard chemotherapy combined with dendritic cell vaccine targeting neoantigen. After identifying neoantigen by liquid biopsy for advanced pancreatic cancer with metastasis, we developed dendritic cell vaccines (neoantigen-DC) in which candidate peptides were synthesized and pulsed to dendritic cells.

Academic Significance and Societal Importance of the Research Achievements

(1)Neo-WT1-DC併用GN療法に関する臨床試験を予定通り全例安全に治療を遂行することができた。治療の結果、RECIST判定でPR7例、SD3例であった。さらに、腫瘍縮小を認めた7例は、手術が可能となった。
(2)neoantigen-DCワクチン併用GN療法を2例実施した。RECIST判定で全例SDであった。尚、治療を実施した1例は、遅延型過敏反応が陽性となった。
以上の新規治療法は、化学療法と併用することで治療効果が増強する可能性が期待された。安全で有効な治療法の開発は社会的意義がある考えられる。
