Outline of Research at the Start

世界的な課題の一つであるフレイルに対し,効果的な予防策が求められている.
本研究では,わが国唯一の成人・高齢双生児のレジストリを用い,フレイルの遺伝率を推計し,関連要因を明らかにすることを目的である.まずは,アンケート・検診データを用いて遺伝率を推計する.次に国内・海外の既存の双生児レジストリを用い,フレイルにおける遺伝/環境因子の影響度について国際比較を行う.さらに,在外日本人,在日外国人への調査を行い,地域環境・人種間比較を通じて,フレイルのより詳細な遺伝的要因,環境要因の影響度を明らかにし,わが国独自のエビデンスの強化につなげるともに,他国に適合できる形を検討し,効果的な予防策を開発する.

Outline of Final Research Achievements

Frailty prevention is a global priority; however, genetic and environmental factors affecting frailty among Japanese people have not been identified. This study conducted face-to-face and postal surveys of identical twins to investigate frailty and its related factors. Our results showed that gait speed, an important indicator for assessing frailty and sarcopenia, had lower heritability than other measurements. Similarly, the phase angle, which reflects cellular health, showed low heritability. We suggested that both indicators could be more sensitive to improvement through interventions.

Academic Significance and Societal Importance of the Research Achievements

本研究は、日本人高齢者のフレイルやサルコペニアに関連する要因に対する遺伝・環境要因の影響を双生児法で初めて明らかにしました。歩行速度や細胞機能を示すPhase angleは遺伝の影響が比較的小さく、生活習慣などの介入による改善が期待されることが示唆されました。本成果は、フレイルの予防において、画一的でない対応の必要性を学術的・実践的に示すものです。
