児童虐待のリスク要因が報告され、子育て支援の取り組みがなされているが、虐待件数は
増加の一途を辿っている。2009年に特定妊婦の支援まで広がっている。このように特定妊婦への関わりが求められているなか、産後の母親への専門職の援助スキルについて、精神科既往妊婦や虐待リスクの母親への対応については自己評価が低く、対応が困難であること、家庭訪問に携わる看護師は母親の抱える抑うつや不安の問題に直面しうまく対処できないと感じていることを報告している。
そこで、特定妊婦への助産師の支援内容や支援の困難さなどの問題を明らかにすることで、特定妊婦への支援スキルが向上し、児童虐待の予防につながることが期待できる。

| Outline of Final Research Achievements |
This study aimed to describe the issues of specified expectant mother and provided supports and recognized issues among midwives. We gave semi structured interview to 10 midwives visiting home of the mothers in a certain city. The steps for coding and theorization (SCAT) analyzed their narratives.
Young pregnant mothers had unstable social backgrounds such as lack of social habits, economical shortage, poor healthcare, and family history of abuses. They tended to set priority to their own will rather than childcare since they aged younger.Mothers with Mental Disorders had difficulty in managing physical and mental health, communication disorder, lack of renting skills to protect their children’s lives, difficulty in creating an environment suitable for care, lack of family support, and financial instability caused by inability to work.

| Academic Significance and Societal Importance of the Research Achievements |
児童虐待のリスク要因が報告され子育て支援の取り組みがなされているが、虐待件数は増加の一途を辿っている。特定妊婦への関わりが求められているなか、産後の母親への 専門職の援助スキルなどの対応については自己評価が低く、対応が困難であること、家庭訪問に携 わる看護師は母親の抱える抑うつや不安の問題に直面しうまく対処できないと感じていることを報告している。今回の研究で、特定妊婦の中でも若年特定妊婦や精神障害のある特定妊婦の問題が明らかになったことで、訪問助産師の支援内容や支援の在り方の方向性が見いだされ、特定妊婦への支援スキルが向上し、児童虐待の予防 につながることが期待できる。
