介護予防事業に参加した高齢者が、終了後も地域のなかで主体的に活動できるよう、住民、自治体、関係者が協働して地域の介護予防への取り組みを持続・発展させていくためには、エンパワメントに着目した戦略的な働きかけが必要と考える。本研究では、コミュニティ・エンパワメントの支援指針および実践現場に求められる体制づくりを明らかにし、介護予防支援者が、地域のソーシャルキャピタルが醸成されるサイクルを生み出す介護予防実践を推進できる実践支援ガイドの開発に取り組む。

| Outline of Final Research Achievements |
The aim of this study was to develop a practical support guide to facilitate community empowerment through the promotion of preventive care activities by community health professionals.
The finalized guide consists of two main components: "Support for Empowerment-Based Preventive Care Activities" and "Aspects of Empowerment".The "Support for Empowerment-Based Preventive Care Activities" section includes:
supporting participating older adults, supporting groups and communities, partnerships between residents and professionals and organizational structure.These elements are presented according to the different stages of preventive care activities.The "Aspects of Empowerment" section illustrates the components and relationships of: participating older adults, organizations and community.

| Academic Significance and Societal Importance of the Research Achievements |
すでに住民主体の介護予防の取り組みを地域活動に発展させ、成果をあげている自治体の報告がいくつかみられている。しかしながら、住民主体の介護予防の取り組みを継続し、自助・互助を基盤とした地域づくりに発展する介護予防実践の方略は未だ確立されてはいない。コミュニティ・エンパワメントを基盤としたガイドが作成できた点において、学術的意義があるといえる。また、介護予防活動の段階に応じて、求められる支援内容および実施体制を明示できた点で社会的意義があるといえる。
