| Outline of Research at the Start |
本研究では，アクションリサーチという研究手法を用い，中等度・重度認知症高齢者に対する「生活の中での自己決定支援」「立つ・歩くなどの軽運動支援」を通して研究者と実践者が協働し，認知症ケアの変化を目指す。
本研究は２段階で構成する。第１段階では研究チームのディスカッションと,症数事例への介入によって介入方法，評価方法を検証する。第２段階では，看護・介護スタッフと協働し，スタッフの日常生活援助（選ぶこと，立つ・歩くことの援助）に対する意識・ケアの変化を促し，継続的な支援実施を目指す。支援の継続が，認知症高齢者の認知機能障害の改善，身体機能の維持・改善，生活の質の向上に繋がると考える。

| Outline of Final Research Achievements |
he purpose of this study is to identify issues in the practice of choice support and activity support for elderly people with dementia, to consider measures to resolve these issues, and to aim for changes in dementia care through these measures. A research team was formed consisting of researchers and practitioners from collaborating facilities, and the study was conducted. A survey of 22 nursing and care staff members revealed that staff members were intentionally creating opportunities for choice and activities through daily efforts. Challenges identified included difficulties in providing choice support when verbal communication is challenging and balancing activities with safety considerations. The study will establish forums for staff to discuss the nature of choice and activity support and evaluate the effectiveness of such support.

| Academic Significance and Societal Importance of the Research Achievements |
本研究により、認知症治療病棟での日常生活における選択支援や活動支援の実施状況、および実施する上での課題が抽出され、課題解決に向けた取り組みを実施するための基盤を構築することができた。認知機能や身体機能の低下した認知症高齢者に対して、看護・介護スタッフにより実施される日常生活援助の在り方を検討し、効果的な支援方法を明らかにしていくことは、認知症の人が最期まで尊厳をもって暮らすための支援方法を示すこととなる。
