| Outline of Research at the Start |
近年，地域包括ケアシステムの構築が推進されている．入院直後から始める退院支援は，病棟において基本的な看護として定着してきているが，いまだに患者は退院後の生活に困難や不安を抱えている．そのため，地域の要援護高齢者が通院する中規模病院の外来で，退院後に継続して在宅療養支援を行うことが重要である．しかし，外来看護師は病棟看護師よりも，在宅療養支援が必要な患者の早期把握の到達度が低く（坂井，2011），外来での在宅療養支援の実施率が低い現状がある．そこで，本研究の目的は，地域の要援護高齢者が通院する中規模病院の外来看護師が在宅療養支援を行うために必要な看護実践能力の自己評価尺度を開発することである．

| Outline of Final Research Achievements |
This study developed a reliable and valid role recognition scale for home care support; specifically, the scale consisted of 15 items across the following three factors: predictive assessment role (six items), relationship role (five items), and communication role (four items). Outpatient nurses in medium-sized hospitals (fewer than 100～200 beds) can use this scale to objectively clarify their own roles in home care support. Notably, such role recognition will enable outpatient nurses to more proactively engage outpatients, which contributes to home care support.

| Academic Significance and Societal Importance of the Research Achievements |
本研究は，中規模病院に勤務する外来看護師の在宅療養支援における役割認知尺度を開発し，信頼性と妥当性，さらに，開発した尺度の実用性を検証した．この尺度を活用することによって，外来看護師が在宅療養支援の役割を認知し，外来通院患者に主体的に関わることができる．また，自己が認知している在宅療養支援の役割を客観的にとらえることに繋がり，課題を明確にすることや，認知できていない役割の想起に有効である．その結果，地域に密着した中規模病院の外来での在宅療養支援が実践されることで，外来通院患者が安心して住み慣れた地域で暮らしを継続していくことが可能となる．
