Outline of Research at the Start

潰瘍性大腸炎（Ulcerative colitis: UC）は、若年者を中心に幅広い年齢層に発症する炎症性腸疾患であると同時に、発癌のリスクを有する難治性疾患である。患者数は本邦のみならず世界的に増加しており、これまで精力的にその病態解明を目的とした研究がなされてきたが、未だに満足のいく結果は得られていない。最大の問題点は、その原因が不明であるということである。申請者は、本研究でUCの自己抗原および病因性自己抗体の同定と、これに基づいた疾患特異的診断法の開発および治療標的の探索を行う。

Outline of Final Research Achievements

We searched for specific antigens and antibodies for ulcerative colitis of unknown cause and identified integrin αVβ6, which is highly expressed in the intestinal epithelium of the colon, and anti-integrin αVβ6 antibody, a specific antibody against it in serum.
The anti-integrin αVβ6 antibody not only had an extremely high diagnostic performance with a sensitivity of 92% and specificity of 94.8%, but also significantly correlated with disease activity during the course of treatment.
The anti-integrin αVβ6 antibody also had the ability to inhibit the binding of fibronectin, an extracellular protein that is the main ligand for integrin αVβ6.

Academic Significance and Societal Importance of the Research Achievements

本研究は、これまで原因不明とされたきた潰瘍性大腸炎の病態解明にせまる発見である。
この発見によって、これまで非特異的な項目から形成される既存の診断基準に対して、疾患特異的な診断マーカーになり得る。特にこれまでの診断項目の中には、患者に身体的および精神的負担を伴う下部内視鏡検査が含まれていた。抗インテグリンαVβ6抗体が診断基準に組み込まれれば、下部内視鏡検査を用いずとも診断できる可能性がある。また炎症性腸疾患の非専門医でも簡便かつ特異的に診断ができる可能性があり汎用性が期待できる。
さらに疾患の病勢とも相関しており、治療効果の判定にも有用な疾患活動性マーカーでもある。
