診療看護師(NP)とは患者の病態と生活を包括的にアセスメントし患者に必要な治療を選択し特定行為ができる看護師である。在宅療養移行支援は、患者の病態の複雑化や在院日数の短縮から医療ニーズの高い患者への支援が増え、看護師の症状マネジメント力が不可欠である。そこで、本研究では①診療看護師(NP)の在宅療養移行支援の実態と②在宅療養移行支援における看護師の症状マネジメントの実態を明らかにした上で、③診療看護師(NP)による症状マネジメントを強化する在宅療養移行支援システムを開発し、その有用性を検討する。

| Outline of Final Research Achievements |
This study aimed to develop a home care transition support system to strengthen symptom management by nurse practitioners, and the following results were obtained. First, an interview survey indicated that the role of nurse practitioners in supporting the transition to home care was to strengthen symptom management by them. Furthermore, using a modified Delphi method, a quality indicator was developed for supporting nurse practitioners in home care transition. Accordingly, a home care transition support system that strengthens symptom management by nurse practitioners was established and discussed with experts, and its validity was suggested.

| Academic Significance and Societal Importance of the Research Achievements |
診療看護師(NP)による症状マネジメントを強化する在宅療養移行支援システムの構築は、早期に在宅療養移行支援に着手し在宅療養を維持するための支援が提供できる。また、患者や家族が在宅療養に慣れるまで診療看護師(NP)が在宅へ出向くこと、訪問看護師と連携し症状マネジメントを行うことで、療養生活が早期に安定し、病気とともに生きる患者・家族のQOL向上に寄与する。開発した診療看護師(NP)が行う在宅療養移行支援に関する質指標を構成する項目は、診療看護師(NP)の在宅療養移行支援における役割認識の実態を把握することや支援の指針として活用できる。
