Outline of Research at the Start

心不全の急性増悪による入院を未然に防ぐには、在宅でのモニタリングが必須である。その手段として、非侵襲的に採取可能な生体由来物質の代表格である尿を用い、ナトリウム利尿ペプチド(NP)を在宅で測定する事を発案した。すでに尿中にNPが排泄されている事は報告されているが、NPには構成分子が複数存在しており、いずれの分子が心不全の“程度”を最も良く反映するのかは未解明である。また在宅での尿中NP測定に適した機器開発も必要である。本研究では、心不全患者由来の尿を用いて心不全の“程度”を最もよく反映する尿中NPを同定するとともに、尿処理条件検討や尿中ナトリウム利尿ペプチド測定高感度・自動化の開発を行う。

Outline of Final Research Achievements

The present study aimed to development a new monitoring system of heart failure utilizing urinary natriuretic peptides. Firstly, we tried to newly develop a sandwich ELISA for measuring urinary N-terminal pro-brain natriuretic peptides (NT-proBNP). Although a commercially available ELISA kit for NT-proBNP could be obtained, some urinary NT-proBNP levels were lower than the sensitivity. Therefore, we treated NT-proBNP with deglycosylation and the urinary levels were two to three folds compared with those without deglycosylation. Moreover, we found that urinary NT-proBNP fluctuates along with heart failure compensation.

Academic Significance and Societal Importance of the Research Achievements

本研究では、将来的な在宅モニタリングの機器開発へ向けて、基盤的研究成果が得られた。本研究で得られた尿中NT-proBNPの濃度分布や、脱糖鎖処理によって変動する濃度、また血行動態と共に尿中NT-proBNPも変動しうる事は、将来的な非侵襲的バイオマーカーの確立や在宅管理への応用に向けて有意義な結果となった。
