| Outline of Research at the Start |
高齢者医療での臨床医の臨床判断支援を目的とする研究である。 具体的な研究目標は、高齢者の慢性疾患(糖尿病、高血圧症、高脂血症、心臓病等)に対する医療介入のタイミングの目安となる臨床判断閾値を高齢者個々の生活・医療背景を反映して推定できる機械学習モデルを作成し、臨床医が治療計画を立て易い情報提供を行うことである。

| Outline of Final Research Achievements |
It is well known that elder patients having one or more three major lifestyle-related diseases (3LD) as diabetes, hypertension and hyperlipidemia would highly have acute heart diseases and cerebrovascular diseases (A-CVD). In this study a signal appearing A-CVD was investigated by use of clinical database. It was found that the number of appearing diseases which are 3LD, chronic kidney disease (CKD), chronic liver disease (CLD) and angina sharply increased before a year appearing A-CVD. Since CKD and CLD show worsening of the whole body and angina does progress of arteriosclerosis, to know a status of whole body so as to speculate the appearance of A-CVD might be equally or more important than to check clinical tests as blood pressure, blood sugar and lipid density.

| Academic Significance and Societal Importance of the Research Achievements |
高齢者において有病率が高い三大生活習慣病(糖尿病、高血圧症、高脂血症)は死因の第二位である急性循環器疾患を引き起こすことはよく知られているが、病態悪化が緩やかなため悪化を早期に発見し、的確に治療を行うことが難しい。本研究で発見された全身病態の悪化を示す疾患の合併数が急性循環器疾患発症近くで急増する傾向があるとの知見は従来の検査値中心の急性循環器疾患発症リスク評価を補う重要な研究成果であり、学術的にも社会的にも意義があると言える。
