Research Abstract

多施設共同の大規模なコホート研究Japan Collaborative Cohort Study (JACC Study)【1988年から90年に40-79歳の男女約11万人を対象に開始】を用いた解析を進めている。本研究では2006年までの死亡追跡により、各部位の死亡者数は、食道がん218例、卵巣がん81例、腎がん(腎盂も含む)99例、膀胱がん148例、骨髄性白血病101例である。また、2001年までの罹患数は食道がん99例、子宮体がん69例、卵巣がん48例、腎がん79例、甲状腺がん118例、多発性骨髄腫31例などである(罹患把握地域が限定されており、死亡に比べ数が少ない)。今年度は女性に多い甲状腺がんにつき生殖歴との関連を検討したところ、妊娠、出産経験があることは甲状腺がん罹患リスクをそれぞれ0.56、0.52と低下させていたが、統計学的に有意ではなかった。その他、初潮年齢、閉経年齢、初産年齢とは関連を認めなかった。また、以前測定したIGF-1、IGF-2、IGF-BP3、TGF-β1、SOD活性、sFasと各部位がんリスクやがんのリスクとなる生活習慣との関連を網羅的に検討した。その結果、歩行時間の長いものではIGF-1が低く、運動が一部のがんに予防的に働くことを説明している可能性が示唆された。また、アポトーシスと関連するsFas値は喫煙者で非喫煙者より高く、BMIが高値になると上昇することを報告した。このことは、炎症、アポトーシスを通じて、喫煙や肥満ががんリスクと関連していることを示唆している。
今年度はさらに2008年までの死亡情報を追加調査した。現在ベースラインデータと突合作業中であり、さらに来年度中には目標である2009年までの追跡を達成する見込みである。なお、各施設と事務局との情報のやり取りには個人識別情報を外しIDを用いている。
