Research Abstract

今年は、筋強直性ジストロフィーやパーキンソン病などの各種神経変性性疾患を対象に、感情認知機能および社会的認知課題を施行し、感情認知障害とその神経基盤を検討した。
筋強直性ジストロフィー(DM 1)症例では、表情感度測定課題を実施した。これと同時に、DM 1症例の有する脳白質病変についてMRIを用いて検討した。脳病変は神経内科医が4段階で評定した。結果として、DM1症例は健常者に比べ、怒り、嫌悪表情の感度に低下がみられた。また、側頭葉前部・島・前頭葉底部の白質に病変がみられた。脳病変と表情感度の関連を調べたところ、怒り表情感度と前頭葉、側頭葉、島病変が、嫌悪表情感度と側頭葉病変が有意な相関を示した。この結果からは、DM1における特有の性格や行動が怒り、嫌悪表情に対する感度低下と関連しており、それぞれ異なる脳病変により引き起こされていることが示唆された。
パーキンソン病(PD)ではこれまでに、ギャンブル課題において短期的に高報酬だが長期的には損をする行動を選択することがわかっていた。この結果は「目先の報酬」にとらわれてしまい、将来損をしてしまうと解釈できるが、これは1.目先の結果にとらわれている2.報酬にとらわれているという2つの要因が関与していると解釈できた。この点について調べるため、ギャンブル課題の正負を逆転させた修正版を用いて、「目先の損失」にとらわれず将来得をすることができるか否かを検討した。結果として、逆転ギャンブル課題においてPD例と健常者との間に選択パターンの差はみられず、いずれの群も有利な山を選択した。すなわち、PD例は逆転課題では目先の高損失にもかかわらず長期的に高報酬な行動を取ることができた。一方版課題においてPD例は目先の高報酬のために的に高損失な行動をとったことを
