Research Abstract

最近、心臓移植や小腸移植の実験研究をみると、キメリズムと免疫寛容の導入についての研究が特に注目されている。平成21年度はキメリズム現象、つまりドナーからレシピエントへの細胞移動についてこれまでの実験を発展させ、新たな結果が得られた。最も興味深い研究は骨髄移植によるキメリズムの誘導である。つまり、臓器移植前にレシピエントに骨髄移植を行いキメリズムの成立、確認した後に目的臓器を移植すれば何の免疫抑制剤を投与しなくても移植臓器が免疫反応を受けることなく生着する。これは移植骨髄のドナーに特異的な免疫寛容であり、小腸など抗原性が高い臓器ですら安定した生着が報告されている。この骨髄移植が免疫寛容を導くならば、四肢に含まれる骨髄は血行を保ったまま移植されることになるため免疫寛容を導かないのかという期待がもたれる。平成21年度はレシピエントに放射線全身照射を行った後にドナーの後肢を同所性に移植した。ドナーにはLacZ Tgラット、レシピエントにはInbred Lewisラットを用いた。この組み合わせだと、何も処置をしなければ移植後4日で移植後肢は拒絶される。MHCでは大きなバリアーがあるペアーである。ドナー骨髄からキメリズムを誘導するために顆粒球刺激因子を投与し、またGVHDを抑制する目的でFK506を28日間投与した。この結果については2009年度のJournal of Orthopedic Researchにすでに報告したが、約20%に高いキメリズムと免疫寛容が得られたが、非致死的な慢性GVHDが認められた。この結果は放射線の量や薬物の使用量によって異なったが、いずれにしろレシピエントに対しては大きな負担となるのは間違いないプロトコールであった。平成21年度は、全身照射量や非骨髄破壊的な前処置について検討すべき結果となった。
