Research Abstract

Body Mass Index (BMI、体重/身長2)は重要な健康関連指標だが、数十年単位の経年的加齢変化を計量的に評価した報告はなく、現状では横断調査(ある時点の調査)から出生コホート(生まれた年)を考慮せずに求めた加齢変化を用いている。一方、国民健康・栄養調査は我が国の代表的な繰り返し横断調査で60年間の記録が存在する。そこで、本調査データを利用し、出生コホートを考慮したBMIの加齢変化を研究した。本年度は、0～25歳男性の結果を欧州肥満学会で、0～60歳代男女の比較を米国の疫学研究会で発表した。前者では、日本人男性は5歳以上ではより最近の出生コホートほどBMIが高いが、コホート間の違いは小児期(特に11歳頃)に顕著であるが、青年期には違いが小さくなることを示した。小児の成長の加速化現象が示唆される。後者では、BMIの加齢変化は男女間で異なり、さらに出生コホート間でも異なることを示した。特に若年成人期に女性はBMIが減少するが男性は上昇する。男性のBMIは、女性に比べ小児初期では高いが、11-12歳頃に低くなり、その後再び高くなる。再交叉する年齢は出生コホートにより大きく異なり、最近のコホートほど若年化している。また、1930年代コホートでは25歳以上の男女差はほとんどなかったが、最近のコホートほど男性のBMIの方が高く成人期の男女差が広がっている。一方、1920年代以前の出生コホートでは男性のBMIの方が低いが小児期の記録はない。世界的に成人および小児肥満が議論されているが、多くの研究は限られた年齢層や、限られた観察年数のデータに基づいている。日本の60年間という長期データにより、幅広い年齢層で出生コホート別変化をみる重要性を示し、従来着目されていなかった「繰り返し年次調査」という研究デザインの利点を示した。加齢変化をみる際には出生コホートを考慮する必要がある。
